The Medication Therapy Management program is designed for people who have several chronic medical conditions or take many different drugs at the same time.
Who Qualifies for the Medication Therapy Management Program?
You will be automatically enrolled in this program if you meet all of the following criteria:
- You take eight or more Medicare Part D-covered medications.
- You have three or more of the following conditions or diseases: bone disease (such as arthritis or osteoporosis), chronic heart failure, diabetes, dyslipidemia, or hypertension.
- You are likely to spend more than the amount listed below on your Part D–covered medications:
