VNS Health Total (HMO D-SNP) brings together Medicare Advantage and Medicaid Managed Long Term Care to help you live safely in your home. The plan offers more than regular Medicare.

VNS Health Total was created for people who want to live in the comfort of their own homes and communities but need ongoing help with daily activities like bathing, cooking, dressing, and walking. Total combines Medicare Advantage and Medicaid Managed Long Term Care benefits into one easy-to-use health plan. Plus, the plan includes extra benefits that aren’t covered by regular Medicare.

Total includes a Flex Card. It’s a debit card you can use to pay for certain utilities and dental, hearing, and vision expenses. You choose how to use it!

Your Own Care Team

As a Total member, you’ll have a team of specialized health professionals who will work with you and your doctor to coordinate your care and connect you with the services you need. Your Care Team is led by a care manager, who is a registered nurse or a social worker.

*This benefit is part of special supplemental program for the chronically ill and not all members qualify. Other types of goods and services not eligible.
