The VNS Health Difference
For more than 130 years, VNS Health has taken care of New Yorkers. Our mission: To improve people’s health and well-being through high-quality, cost-effective health care in the home and community.
Our specialized health plans are one facet of that mission. We offer only Medicare and Medicaid plans, and we’re experts in coordinating the two for people in New York State. Our plans are designed to help people live, age, and heal in their own homes, connected to their families and communities. That’s why they offer the benefits and support our members need now and as their needs change over time.
About Our Plans
Our health plans are about more than just access to health care. They include important services and supports designed to help every member be as healthy as possible.
Each member has access to an interdisciplinary care team, which may include nurses, social workers, pharmacists, or other experts, according to the member’s needs. Care Teams support members and their providers in many ways, including helping members follow their doctors’ plan of care, coordinating health care, and connecting members to community services.
All of our Medicare plans provide valuable extra benefits designed to make healthy living easier and more affordable. These include dental, vision, and hearing coverage along with an over-the-counter (OTC) allowance that helps members afford necessary health items. Our Medicare plans for people with Medicaid also include allowances members can use to buy groceries, help pay for home utilities, and more.
We have long-standing relationships with leading providers, and we know how to work with them as partners in helping their patients – our members – achieve their best health. VNS Health is also a home care provider, so when additional services are needed, we understand how to connect home and community services with office-based care to best support the patient.
Where to Find Us
Service Areas
VNS Health started in New York City. Over the years, we’ve expanded to offer our health plans in more counties across the state, so we can help a growing number of New Yorkers live well in their homes and communities.
Community Locations
Our community centers located in Chinatown, Flushing, and Sunset Park are places where anyone – not only our health plan members – can stop by for support and resources. We also have two neighborhood offices (Flatbush & Far Rockaway) where interested people can speak with an advisor about Medicare plans.
