VNS Health Total (HMO D-SNP) brings together Medicare Advantage and Medicaid Managed Long Term Care (MLTC) to help you live safely in the comfort of your home. The plan offers more than regular Medicare.

Our VNS Health Total plan was created for people who need ongoing help with daily activities like bathing, cooking, dressing, and walking, and who want to live in the comfort of their own homes and communities. Total combines Medicare Advantage and Medicaid MLTC benefits into one easy-to-use health plan. Plus, it includes extra benefits that aren’t covered by regular Medicare.

Flex allowance*

Get $65/month (up to $780/year) towards home utilities, and certain dental, hearing, and vision expenses. You choose how to use it.

$3,500/year for preventive and comprehensive dental care.

    Total includes:

    • One easy-to-use card with both your OTC/Grocery allowance* ($310/month), and Flex allowance* ($65/month) to spend on home utilities and certain dental, hearing and vision expenses. If you don’t spend the full amount in one month, it rolls over to the next, as long as you use the full amount by the end of the year.
    • SilverSneakers® fitness program — with gym membership and online classes. 
    • Dental, hearing, vision, acupuncture, and podiatry coverage.

  • Your Own Care Team

    As a Total member, you’ll have a team of specialized health professionals who will work with you and your doctor to coordinate your care and connect you with the services you need. Your Care Team is led by a care manager, who is a registered nurse or a social worker. 

*Grocery and utility benefits are part of special supplemental benefits for the chronically ill and not all members qualify.
