VNS Health EasyCare Plus (HMO D-SNP) and VNS Health Total (HMO D-SNP) members need to keep their Medicaid coverage active.
Why Is Renewing Medicaid On Time Important?
If you don’t renew on time, you could lose your Medicaid and be disenrolled from your health plan. Medicaid must have your current contact information to mail you your Medicaid renewal notice.
How to Update Your Medicaid Contact Information
Contact your Medicaid office to give them your current address, phone number, and email address.
- If you live in New York State outside of New York City, call your local Medicaid office. To find your office, contact your Local Department of Social Services (LDSS). Search the LDSS directory on the NYS Department of Health website or call the NYS Medicaid helpline at 1-800-541-2831.
- If you live in New York City, call Medicaid at 1-888-692-6166 (TTY: 711), Monday – Friday, 8 am – 5:30 pm.

Watch out for Medicaid scams!
Be alert for scammers who ask for a fee to help you renew or recertify your Medicaid or Medicare coverage. Never give your Medicaid, Medicare, Social Security number, or date of birth to anyone who calls you or sends you a text.
VNS Health and/or your local Medicaid office will send you notices in the mail about renewal. We may send you reminders in the mail to update your contact and income information. We will never ask for money to help you renew or apply for Medicaid.
If you get a text, phone call, or email that you think may be a fraud, contact your local Medicaid office.
