Member Rewards Program

We reward you, so you can reward yourself!

As a member of VNS Health MLTC, you’re automatically enrolled in our Member Rewards Program. You can earn rewards for completing certain health activities, like important exams and vaccinations. Earn up to $25 for each health activity you complete.

You can earn rewards by completing the qualifying activities listed below.

Additional eligibility exclusions may apply. All services must be medically necessary to earn rewards. All health activities are awarded once a year, except where noted differently.

How Will I Get My Rewards?

You must be an active member of the plan to earn and receive rewards. We’ll track your progress using information from your provider to verify that you completed the health activity.

You’ll receive your Rewards card the first time you earn rewards. Your rewards will be loaded onto it. Keep your card—it will be reloaded each reward period. Below is a schedule of when rewards will be issued in 2025.
