Provider Model of Care Training and Attestation Form

Every year, providers in the VNS Health Medicare network are required to take our Model of Care training. There are two steps to completing your training:

1. Download and review the 2023 Model of Care Training.
2. Complete the attestation form below by December 31, 2022, to let us know that you have downloaded and reviewed the 2023 Model of Care Training.
