Medicare Patients May Be Surveyed About the Care You Provide

From July 2024 to November 2024, the Centers for Medicare, and Medicaid Services (CMS) is mailing out the Health Outcomes Survey (HOS) survey to some members of the VNS Health Medicare plans.

The HOS asks about the member’s mental and physical health, physical activity, incontinence, and falls risk and prevention. The survey is used by CMS to assess health plan performance in the Star Ratings Program. Please encourage your patients to complete the survey if they receive it. Members’ answers on these surveys affect our quality scores and reputation, which can drive enrollment and lead to more patients for you.  

If patients have questions, they can call the phone number on the survey.  

Also, as a reminder the Consumer Assessment of Healthcare Providers and Systems (CAHPS) is active until June 2024.
