What is VNS Health Total?

VNS Health Total makes getting health care simple. The plan combines Medicare Advantage and Medicaid MLTC benefits into one easy-to-use health plan. Plus, it includes extra benefits that aren’t covered by regular Medicare.

Our Total plan was created for people who need ongoing help with daily activities like bathing, cooking, dressing, and walking, and who want to live in the comfort of their own homes and communities.

Who is Eligible for Total?

You may be eligible for VNS Health Total if:

You are at least 18 years old

You are eligible for Medicaid

You are eligible for Medicare Part A and Part B

You are in need of long-term care, for more than 120 days, based on a functional assessment (must be eligible for nursing home level of care, as of time of enrollment, based on the Community Health Assessment)

You are able to live safely at home with help for activities of daily living, such as bathing, dressing, walking, and making food.

You live in the VNS Health Total service area: Albany, Bronx, Erie, Kings (Brooklyn), Monroe, Nassau, New York (Manhattan), Queens, Rensselaer, Richmond (Staten Island), Schenectady, Suffolk, and Westchester.
