Long Term Services and Supports
As a Total member, you’ll have a team of specialized health professionals who will help you and your doctor plan your care. Your Care Team is led by a care manager, who is a registered nurse or a social worker. Your care team can help with day-to-day tasks such as bathing, dressing, walking, and cooking.
Long term care services available at no cost to help you live well at home:
- Skilled nursing services or therapies in the home
- Personal care services
- Home health aide services
- Consumer Directed Personal Assistance Services
- Eligible house modifications (e.g. grab bar installed in bathroom)
- Plan-approved supplies and equipment (e.g. wheelchairs, walkers)
