VNS Health Helps Connect Medicaid Members to Community Support

VNS Health is helping Medicaid members connect to local resources for food, housing, transportation, education, and more through New York’s Social Care Networks (SCNs). These services help individuals and families get support for everyday needs and improve overall well-being.

If you are enrolled in Medicaid and you or your family are looking for support with food, transportation, or housing, there may be help available to you.

Medicaid members can reach out to a Social Care Network to discuss their social needs.

For those who may benefit from additional supports, you may work with someone to get connected to the services that are right for you, depending on your health status and need.

You can get help with:

  • Transportation
  • Nutrition
  • Education
