Eligibility

To enroll in VNS Health MLTC you must be:

At least 18 years old

Eligible for Medicaid

In need of long-term care and in-home support for over 120 days based on a functional assessment and meet the following Minimum Need: at least limited assistance with physical maneuvering with more than 2 activities of daily living (ADLs); or individuals with a Dementia or Alzheimer’s diagnosis, assessed as needing at least supervision with more than one ADL

Able to remain safely at home with assistance for activities of daily living (such as bathing, dressing, walking, or preparing food)

Living in the service area of the plan* 

You or a member of your family can call us directly to discuss enrollment in the program. If you need help applying for Medicaid, please let us know.

*The VNS Health MLTC service area includes these counties: Albany, Allegany, The Bronx, Broome, Cattaraugus, Cayuga, Chautauqua, Chemung, Chenango, Columbia, Cortland, Delaware, Dutchess, Erie, Fulton, Genesee, Greene, Herkimer, Kings (Brooklyn), Livingston, Madison, Monroe, Montgomery, Nassau, New York (Manhattan), Niagara, Oneida, Onondaga, Ontario, Orange, Orleans, Oswego, Otsego, Putnam, Queens, Rensselaer, Richmond (Staten Island), Rockland, Saratoga, Schenectady, Schoharie, Schuyler, Seneca, Steuben, Suffolk, Sullivan, Tioga, Tompkins, Ulster, Warren, Washington, Wayne, Westchester, Wyoming, and Yates.
