EasyCare Plus (HMO D-SNP) is a Medicare Advantage plan for people with Medicaid. It includes prescription drugs and extra benefits not covered by regular Medicare.

A Smart Plan for People with Medicare and Medicaid

EasyCare Plus members have a $0 monthly premium and $0 copays (depending on the level of Medicaid eligibility) for health care services. The plan gives you more than regular Medicare, including extra benefits that can make it easier to live a healthy lifestyle in your home and community.

With EasyCare Plus, you get great prescription drug benefits. You pay $0 for all medications including brand names, generics, and insulins.

Regular Medicare covers a lot, but not everything. EasyCare Plus has important benefits that aren’t part of regular Medicare, including prescription drugs, dental, hearing, vision, acupuncture, and much more.

EasyCare Plus includes:
- - One debit card with your OTC/Grocery allowance ($235/month), Flex allowance ($37.50/month), and member rewards. The allowances can be rolled over every month, so you can save up for a larger purchase — just be sure to use the full amount by the end of the year.
  - SilverSneakers^®fitness program. Makes it easier to be active with gym membership and online classes. Many activities can be done from the comfort of your home.
  - Get to the health care locations you need with 7 round trips to and from approved facilities.

Personal Support

With EasyCare Plus, you get a health coach or care manager to help you be as healthy as possible. Our plan pharmacists can make it easier for you to get and take your medications. You can have doctor’s visits in the comfort of your home with our telehealth program. And you have 24/7 access to a nurse for when health concerns arise.

*Benefits and costs depend on your level of LIS or Medicaid eligibility.
